Do I need the + sign before the country code?

No. Remove the + and enter the number starting with the country code. For example, +1 415 555 2671 becomes 14155552671.

Is the pre-filled message sent automatically?

No. The message appears in the chat input field, but the user must press send in WhatsApp to actually send it.
